BCHC Modernizing Healthcare Improves Patient Care

ALBION, Neb. (Dec. 10, 2025) – Boone County Health Center (BCHC) is earning nationwide recognition for transforming how it delivers care through advanced population health technology. This modernization is improving outcomes for patients across the Health Center’s seven-county service area.

Over the past two years, BCHC partnered with i2i Population Health, an Azara Healthcare company, and Oracle Health to move from manual spreadsheets and time-consuming chart reviews to a real-time data platform. This upgrade helps care teams track preventive care, chronic conditions, and follow-up needs during everyday patient visits.

“Our goal is simple—use data to drive smarter care, support our clinicians, and make sure every patient gets the right care at the right time,” said Beverly Nelson, RN, Clinical Quality and Population Health Manager at BCHC. “This new system finally gives us the tools and accuracy we need to make that happen.”

The improved system gives providers instant access to accurate, up-to-date patient information. Patients are more likely to stay current on screenings, lab work, and routine preventive care. Providers can prepare for appointments more efficiently, staff spend less time on paperwork, and high-risk patients can be identified earlier for timely support.

Since adopting the i2iTracks platform, BCHC has achieved measurable gains, including perfect Blue Cross Blue Shield quality scorecards, increased shared savings through Accountable Care Organization contracts, and strong improvements in major quality measures from 2022 to 2024. The system has also reduced the hours required for manual data entry and strengthened provider confidence in data accuracy. These improvements have positioned BCHC as a leading example of how small rural hospitals can use data-driven tools to improve patient care and long-term sustainability.

Looking ahead, BCHC plans to expand its use of advanced analytics to support risk stratification, referral management, and cost tracking across its patient population. The next phase will integrate i2iPRiZiM analytics, giving leadership deeper insight to align clinical quality with financial performance.
