Boone County Health Center’s commitment to care doesn’t end when you leave the doctor’s office or hospital. There are times in your life when you may need health care at your home. We are here for you with five highly trained visiting nurses, one CNA, one medical social worker, and physical/occupational and speech therapists and assistants, who provide compassionate, quality care in the comfort of your own home. Home health provides individualized care for patients with a medical condition or patients who are recovering from surgery.
