Boone County Health Center’s commitment to care doesn’t end when you leave the doctor’s office or hospital. There are times in your life when you may need health care at your home. We collaborate with your physician to create a personalized care plan that supports your recovery goals. Our nursing and therapy professionals provide intermittent services based on that plan, with a strong focus on keeping you safe and healthy at home. Our priority is to help you recover successfully while avoiding unnecessary hospital stays.
