We’re Making Sports Physicals Easier!

We’re making it easier for local athletes to get their sports physical by offering a Convenient Sports Physical Clinic.
No appointment is needed!
Clinics will be held:
ALBION: July 19th from 4:30-6:30 p.m.
ELGIN: July 21st from 2-4:30 p.m.
SPALDING: July 12th from 2-4:30 p.m.
FULLERTON: July 11th and July 20th from 3:30-4:30 p.m.;
NEWMAN GROVE: July 18th from 2:30-4:30 p.m.
These are the only times sports physicals will be offered for $25. All other sports physicals will be done at regular price.
Student athletes need to fill out their school’s sports physical form ahead of time and bring it along with their immunization records. They should also bring their glasses and contacts.
To speed up the process, student athletes can also bring a filled out BCHC registration form found online here: Registration Form (or at any clinic) as well as a urine sample. Urine cups can be picked up ahead of time at the clinic. If this is not possible, it’s important to come with a full bladder.
There will be no submission to insurance and no immunizations will be given at the Convenient Clinic. Any additional services will need to be scheduled at a different time.
